When receiving an enquiry Medical Assessment is conducted by the clinical team to determine the nursing and care needs of the patient. Following the assessment an Individualized Care Plan is developed with the patient’s family with goals and outcomes to improve the patient’s conditions, enabling the Care Professionals to carry out their service effectively or as per the directive of the Physician/ Family Doctor. As needed, the family doctor / physician is consulted.

MyCare Medicare has registered nurses who are specialized in the treatment of specific conditions. These nurses render nursing care as per established standards effectively. Our RNs will visit the patient at their home at a pre-fixed time and fulfill the care needs of the patient.
